VERIFICATION REQUIREMENTS FOR ACCREDITATION OF LAND-BASED PRINCIPAL/S
Documentary Requirements:
1. Manpower Request/Job Order (stating request for manpower from Philippine agency and stipulating the positions, number of worker required, salaries, duration of contract, etc.)
- Must be notarized by a Solicitor/Notary Public in UK/Northern Ireland and thereafter legalized by the UK Foreign and Commonwealth Office (FCO).
2. Special Power of Attorney
- Must be notarized by a Solicitor/Notary Public in UK/Northern Ireland and thereafter legalized by the UK Foreign and Commonwealth Office (FCO).
3. Recruitment Agreement
- Must be notarized by a Solicitor/Notary Public in UK/Northern Ireland and thereafter legalized by the UK Foreign and Commonwealth Office (FCO).
4. Master Employment Contract
- Must be notarized by a Solicitor/Notary Public in UK/Northern Ireland and thereafter legalized by the UK Foreign and Commonwealth Office (FCO).
Note: Signatory of documents 1-4 should be consistent/the same.
5. Certified true copy of Company’s Certificate of Registration/ Incorporation or data page of employer’s passport.
6. Proof that the company is licensed by the UKBA/RI to sponsor migrant workers.
7. Copy of POEA Valid License of the Philippine agency or, if still applying, copy of certification from POEA that the Philippine agency is a prospective applicant.
8. Other supporting documents, as may be required by the Office of the Labor Attache.
Procedures & Fees:
Personal Transaction
1. Present the above documents with one (1) photocopy set to the Office of the Labor Attache, Philippine Embassy;
2. If found complete and in order, the same will be processed, stamped, verified and signed;
3. Pay the verification fee of £14.00 each for the: (i) manpower request/job order; (ii) Special Power of Attorney; iii) Recruitment Agreement, (iv) Master Employment Contract; (v) Employer’s certificate of registration/incorporation; (vi) other documents.
4. Payment should be done in cash;
5. Present the documents to the Consular Section, Philippine Embassy, and pay the authentication fee of £18.00 for each of the verified documents; and
6. Wait for the release of the documents.
By Post
1. Send the above documents with one (1) photocopy set to the Office of the Labor Attache, Philippine Embassy, 6 Suffolk St, London SW1Y 4HG;
2. Include payment, in postal money order, payable to “POLO-London” for verification fees which is £14.00 each for: (i) manpower request/job order; (ii) Special Power of Attorney; iii) /Recruitment Agreement, (iv) Master Employment Contract; (v) Employer’s certificate of registration/incorporation; (vi) other documents.
3. Include another payment, in postal money order, payable to “Philippine Embassy-London” for authentication fees which £18.00 for each document as above.
4. If found complete and in order, the same will be processed, stamped, verified and signed;
5. Enclose a self-addressed stamped envelope for return of documents.

1. Crew Complement (stating request for manpower from Philippine agency and stipulating the positions, number of worker required, salaries, duration of contract, etc.)
- Must be notarized by a Solicitor/Notary Public in UK/Northern Ireland and thereafter legalized by the UK Foreign and Commonwealth Office (FCO).
2. Special Power of Attorney
- Must be notarized by a Solicitor/Notary Public in UK/Northern Ireland and thereafter legalized by the UK Foreign and Commonwealth Office (FCO).
3. Manning Agreement
- Must be notarized by a Solicitor/Notary Public in UK/Northern Ireland and thereafter legalized by the UK Foreign and Commonwealth Office (FCO).
4. Master Employment Contract with Standards Terms and Conditions of Employment
- Must be notarized by a Solicitor/Notary Public in UK/Northern Ireland and thereafter legalized by the UK Foreign and Commonwealth Office (FCO).
Note: Signatory of documents 1-4 should be consistent/the same.
5. List of ships/vessels and particulars, including IMO number, gross tonnage, flag, etc.
6. Certified true copy of Ship/Vessel Registry
7. Certified true copy of Company’s Certificate of Registration/ Incorporation or data page of employer’s passport
8. Proof that the company is licensed by UKBA/RI to sponsor migrant workers
