Shipping of Human Remains or Ashes to the Philippines
For Human Remains:
- A. Coroner’s Certificate of the Fact of Death or in its absence, an Interim Death Certificate issued by the Coroner’s Office
- B. Embalming Certificate issued by the Funeral Parlor
- C. Free from Infection Certificate from the concerned hospital or from the local funeral parlor
- D. Out of England Removal Certificate from the concerned UK authorities
- E. Copy of the data page of the deceased’s passport
- F. Fee of £18.00 upon issuance by the Philippine Embassy of a Consular Mortuary Certificate
- G. Full repatriation details and consignee’s address
For Cremated Remains:
- A. Death Certificate
- B. Cremation Certificate
- C. Copy of the data page of the deceased’s passport
- D. Fee of £18.00 upon issuance by the Philippine Embassy of a Consular Mortuary Certificate
- E. Full repatriation details and consignee’s address
